Over time, discharge planning has evolved beyond simply moving patients out of hospitals. Today, discharge planning processes begin as soon as patients arrive and go well beyond discharge. CMS recently enhanced its take on discharge planning. Are you familiar with the agency's new position? Would your facility pass the next survey? On July 13, AHC Media will host a webinar that will help attendees understand modern discharge planning processes.

During this session, our experts will explain CMS' latest process as well as strategies for safely transitioning patients across the continuum of care. Additionally, we'll discuss how to engage other members of the interdisciplinary care team in the process of planning patients' movements across the continuum, including verbal and written hand-off communication.
