Special: Avoiding penalties for readmissions

Case managers are a hospital's first line of defense when it comes to smoothing transitions of care and preventing readmissions. For the next two months, Hospital Case Management will focus on how you can help your hospital avoid the penalties that the Centers for Medicare & Medicaid Services plans to levy when patients are readmitted within 30 days. In this issue, we look at how to analyze your readmissions to identify gaps in care and focus on ways to help patients stay safe at home. In the March issue, we'll discuss why it's important to assess a patient's health care literacy, how to do it, and how to look at ways to ensure that providers at the next level of care have the information they need to meet the needs of your patients.
