Physician Care Coordination Procedure
Policy: The clinical record establishes that effective interchange, reporting and coordination of client care does occur.
Purpose: Communication of significant and ongoing clinical data shall be reported to the physicians’ office professional staff through regular visits by the agency coordinating registered nurse.
Procedure:
Responsible Person: Primary Nurse
Action: 1. Will complete specific flow sheet after each client visit.
2. Will submit flow sheet to coordinating nurse weekly.
3. Will submit medication to coordinating nurse at start of care and update with every medication change.
4. Will document signs/symptoms and concerns in comment section of flow sheet.
Responsible Person: Coordinating Registered Nurse
Action: 1. Will visit physician’s office weekly and conference with professional staff.
2. Will provide updated information for primary nurse.
Responsible Person: Physician and/or Professional Office Staff
Action: 1. Will review information provided and amend Plan of Care if appropriate.
Responsible Person: Primary Nurse
Action: 1. Will request non-emergency orders/new prescriptions/referrals from physicians through coordinating registered nurse.
