Pain Management in the Emergency Department: Opioids and Alternative Pain Management Therapies
Opioid therapy can be an effective form of pain management in the ED for acute painful conditions. The risk of addiction and abuse should be considered in every case. Alternatives to opioid therapy include systemic agents, such as acetaminophen, NSAIDs, lidocaine, alpha agonists, anticonvulsants, ketamine, corticosteroids, and local and regional anesthesia.

These investigators found that the routine use of acetaminophen alone rather than a combination acetaminophen-opioid significantly reduced overall and daily opioid use. In addition, there was no worse effect on overall pain score or length of stay.

Acetaminophen for Fever in the ICU
SYNOPSIS: Seven hundred ICU patients with fever and known or suspected infection were randomly assigned to receive acetaminophen 1 g IV or placebo every 6 hours until ICU discharge, resolution of fever, cessation of antimicrobial therapy, or death. Early administration of acetaminophen did not affect number of ICU-free days.
