It was a big step for CMS to recommend physicians be compensated for advance care planning. Better end-of-life care can result in reduced readmissions. It is important to engage with the patient on what matters to them at end-of-life care and to respect their wishes for care.
The primary purpose of this webinar is to cover in detail the CMS hospital CoPs and TJC standards on advance directives to ensure compliance. This program will cover advance directives such as living wills, durable power of attorney, organ donation, mental health declaration, organ donor cards, patient advocates, and do not resuscitate orders. It will discuss rights of patient representatives such as DPOA, support persons, parents, or guardians.
The presentation will provide you with the tools you need for asking patients about end of life wishes, the details of the IHI conversation starter kit, resources on advance care planning and the use of CriSTAL to identify dying patients.
