Over the past several decades, professional case management associations have identified the importance of effective transitions throughout the patient’s continuum. Effective transitions are wholly dependent on successful communication. Part of successful communication is an effective hand-off between transitioning providers of care.

Recently, CMSA and the ACM have more definitively described the case manager’s role in effective transition management.
