Save 20% with the Case Management Across the Continuum of Care Series!

Over the past several decades, professional case management associations have identified the importance of effective transitions throughout the patient’s continuum.  Effective transitions are wholly dependent on successful communication. Part of successful communication is an effective hand-off between transitioning providers of care.

It is equally important that case management and nursing focus on effective hand-offs. Recently, CMSA and the ACM have more definitively described the case manager’s role in effective transition management. During this webinar, we will review CMSA’s recommendations and the role of both the RN and social work case manager’s communication and hand-off as they effectively and efficiently transition hospital patients through the hospital and into the post-acute care continuum.

Do your policies address the complexities of the new healthcare environment?

Call us at 800-688-2421 or add this event to your cart above.

    Covered Topics:

    - Rounding - Facilitation & coordination - Communication & collaboration
    - Effective CM teams - Hospital-isolated silos - Statement of philosophy
    - Written vs. verbal - Effect on Medicare spending  - Time out as an effective tool
    - Standards of practice - Multiple healthcare providers - Post-acute care isolated silos
    - Guiding team principles - Professional roles & responsibilities And that's just the beginning!

After this webinar participants will be able to:

    1. Develop a dashboard to review effectiveness of communication and handoff transitions.
    2. Explain the professional responsibilities of the RN and social work case manager in internal and external transition communication and handoffs.
    3. Explain new and revised case management standards, regulations, and laws put forth by CMS, TJC and the federal government.
    4. Evaluate case management protocols and penalties.
