One in five patients discharged from a hospital experience an adverse event within 30 days, often leading to emergency department trips and readmissions. Research suggests that up to 75% of these readmissions could be preventable, saving hospitals $528 million dollars in Medicare reimbursement this year alone.
Attendees will learn about the updated Project Re-Engineered Discharge (RED) and strategies to reduce readmission. Our speaker will cover newly proposed discharge planning standards as well as mandatory changes implemented with the IMPACT Act.
