Questionnaires evaluate lifestyle changes
Lifestyle changes require extensive education that is best done in an outpatient setting after discharge. Health outcomes assessments can help patient education managers keep these outpatient programs on track, says BJ Hansen, BSN, patient education coordinator at Grant/Riverside Methodist Hospitals in Columbus, OH.
Following are a few of the questions used to evaluate the effectiveness of the hospital systems’ outpatient cardiac rehab program. Patients are asked to indicate the frequency they engage in each behavior:
• Choose a diet low in fat, saturated fat, and cholesterol.
• Follow a planned exercise program.
• Get enough sleep.
• Eat three to five servings of vegetables each day.
• Practice relaxation or meditation for 15-20 minutes daily.
• Read labels to identify nutrients, fats, and sodium content in packaged food.
• Check my pulse rate when exercising.
• Balance time between work and play.
• Do stretching exercises at least three times per week.
• Ask for information from health professionals about how to take good care of myself.
